What are hemorrhoids?

The "Hemorrhoid disease“ is a widespread disease that nobody talks about. The problem: Conventional hemorrhoid operations can be very painful, so the fear is justified. Hemorrhoid surgery can also take a long time to heal. Hemorrhoids are nodular enlargements of the rectal vessels. Normal anal vessels serve to regulate intestinal activity. Stretched rectal vessels are “like varicose veins in the rectum” and cause the intestinal wall to prolapse. Weeping, skin irritation, burning, itching, stool smearing, partial incontinence and even bleeding will occur during the increase in hemorrhoids.

Causes of Hemorrhoids?

The cause of hemorrhoids is not exactly clear. The main cause seems to be the civilized way of life. Modern man sits too much and consumes too little fiber. Muesli, salad, oatmeal, wheat bran are part of a conscious and good diet that not everyone follows. But genetic predispositions can also be the reason for hemorrhoids.

Symptoms and degrees of severity in hemorrhoids

Hemorrhoids develop progressively and are classified from severity I-IV. They differ by their symptoms.  hemorrhoids im Stage I are only perceptible to the proctologist. In the stage II the hemorrhoids protrude in front of the rectum during bowel movements. In the  Stage III the prolapsed hemorrhoids no longer retract spontaneously but must be pushed back with a finger. Finally in Stage IV  there is a persistent prolapse from the rectum. Hemorrhoids of any severity often cause bleeding and itching in the anal area. In addition, there are often voiding disorders. Patients with hemorrhoids have to go to the toilet several times in a row and are unable to urinate all at once. Constipation, feeling of pressure, pain are often symptoms. The involuntary passage of stool or smearing stool, traces of stool on underwear occurs in advanced stages, especially with diarrhea. Finally, hemorrhoids cause filled so-called perianal veins, which are perceived as “varicose veins of the rectum” or external hemorrhoids. These are problematic because a perianal thrombosis or anal vein thrombosis follows or small tears or bleeding painful anal tears develop, which then - because of the underlying disease haemorrhoids - heal poorly. The heavily congested perianal veins do not allow the anal entrance to be completely sealed, so the internal hemorrhoids should be reduced or eliminated to ensure complete tightness and dryness of the anal entrance. 

Laser therapy for hemorrhoids

The minimally invasive (Laser hemorrhoid plasty or LHP) relies on the natural regression of hemorrhoids after laser irradiation of the supply arteries. The symptoms should recede quickly so that the patients can continue to work and socialize after the procedure. There are no open wounds and the sensitivity and the sphincter remain fully intact.

The Laser Hemorrhoids Plastic Surgery (LHPC) is the modification, further development of the LHP. Principles, laser-technical work steps, instruments and substrate of the therapy were adapted by dr(H) Haffner to his own clinical experiences. LHPC is a further development of laser haemorrhoid plasty (LHP).

 In laser hemorrhoidal plastic surgery, the focus is not only on the supply artery, but much more on the entire arterial and venous anatomy of the anal "corpus cavernosum". The principle, the approach, the implementation and the instruments in the laser treatment of hemorrhoids - LHPC - were presented by Dr. Haffner changed. Laser Hemorrhoid Plastic Surgery expresses the radical and advanced laser application of the entire hemorrhoid cushion. It is an advanced and optimized laser therapy for hemorrhoids without cuts, without lacerations, without scars and almost without pain. All the usual surgical incisions, cutting out hemorrhoids, large and deep wounds and the associated pain are avoided with laser treatment of hemorrhoids. With our type of laser therapy for hemorrhoids, the hemorrhoids shrink considerably even on the operating table, the rectum looks much denser, much more closed than before the operation. DR. Not only do we want to hope that the hemorrhoids will go away as a result of the laser treatment, we want to be able to see and do it right away on the operating table. The effectiveness of Laser Plastic Surgery (LHPC) by dr. Haffner is recognizable by the immediately visible shrinkage and radical reduction of all hemorrhoids and by means before and after pictures demonstrable. Such a degree of hemorrhoid reduction is not possible with surgical incisions. Because you can't cut out and remove everything from the rectum, there would be no mucous membrane left over. Likewise, the external hemorrhoids cannot be cut out, because then the total numbness of the anal entrance, the so-called sensory incontinence according to Whitehead, would occur. With laser hemorrhoids plastic surgery, both the inner and the outer hemorrhoids - the perianal veins - can be removed with very high effectiveness and hardly any side effects. 

What are the side effects of hemorrhoid laser surgery?

Of course, laser beams can also be harmful. Apparently, this is why laser therapy for hemorrhoids has become so widespread, because it aims to achieve high effectiveness with minimal side effects. This means: even in the case of large and circularly extended and protruding hemorrhoids that extend to the entire rectum without cutting, only the laser beam can be used to significantly reduce and the damage to the mucous membrane caused by the heat of the laser beam can be reduced to a minimum. That is why it is not the method (LASER) that counts, but the way the laser method is used, i.e. know-how and experience. With increasing experience, we have therefore developed our own LHPC method, because we can achieve good results with it in practically all stages of haemorrhoids plus perianal veins, thrombosis and skin tags, which can be treated with other methods (LHP, HAL, RAR, Longo- method, Milligan Morgen, Parks, Ferguson, etc. methods) of hemorrhoidal surgery probably does not work. We have also worked with the above conventional methods before and we also started with the LHP laser method. After several hundreds of laser surgeries, all conventional surgeries with knives and scissors have been omitted and found that these are neither necessary nor up-to-date. Since the introduction of laser therapy for hemorrhoids, we have not had any more emergencies after hemorrhoid surgeries: no more emergency calls, no more need for an ambulance, no need to transport the patient to the hospital, no major bleeding, no abscesses like after hemorrhoid operations. The sphincter is also protected, because the laser beams in the LHPC - and not in the LHP !!! – do not injure the sphincters at all. LHPC summary: 

The new LHPC provides an expanded, more complex version of the LHP. Both the principle, the approach and the instrumentation are different, further developed compared to the original LHP method. With the LHPC, the otherwise usual rectal incisions, which are made in other haemorrhoid operations, are completely avoided. Post-bleeding, risks of abscesses, and risks of sphincter damage hardly exist with LHPC, while the above-mentioned side effects and complications often occur after classic operations. Patients can walk, sit, and have no pain immediately after LHPC. Minimal swelling and sometimes small tears and superficial mucosal wounds are the minimal, very well tolerable side effects of LHPC and only if the intervention is exceptionally carried out on several sectors, with higher power and at a later stage. We follow the conservative strategy: rather several small, risk-free interventions than one major one with complications. As a result, there are hardly any surgical wounds, no surgical pain and no postoperative infections, no disruptions to surgical wound healing and, in particular, no damage to the sphincter muscles. In particular, anyone who opts for a classic haemorrhoid operation, removal with a knife, scissors, etc., should consider the damage to the sphincter muscles with the risk of incontinence and disability for life. Women describe the pain after classic hemorrhoid operations as worse than the birth of their child. Haemorrhoid removal is usually particularly gentle. In contrast to conventional treatment, there are usually no bleeding wounds, swellings or suppurations. With the laser, the hemorrhoids are "welded" by heat. The affected vessels, which are stretched out - the "varicose veins of the rectum" - are shrunk in this way and regress. If there is already a prolapse of hemorrhoids, then we apply further, incision-free, minimally invasive methods hemorrhoid treatment and mucosal tightening (RAR) in order to be able to tighten the rectum immediately during the operation without incisions or wounds.
