Laser Pilonidal Sinus Plastic Surgery (LSPC)

An Pilonidal sinus causes permanent, recurring purulent inflammation in the bottom fold. In the acute stage, redness, tenderness, pain and fever can develop. If the coccyx sinus breaks out, there is a constant flow of pus, weeping in the buttocks and itching. A coccyx fistula laser surgery should therefore be carried out promptly.

Which method for coccyx fistulas?

Which is the best method for me? Most patients ask this question – and rightly so. But it's not just about the best method, it's about the stage at which a coccyx fistula, a coccyx abscess is located. Is it an abscess with paptem red and very painful area around the abscess, then the first therapy should be limited to relieving the symptoms in a minimally invasive way. For this purpose, the LSPC procedure - Laser Steinßbein Plastic Surgery, which has been tried and tested by us, will be carried out, which is supplemented with laser sealing and vaporization pit picking method after I Iesalnieks 1 represents.

The Laser Pilonidal Sinus Plastic (LSPC) Surgery

is a gentle, incision-free method in which healing can take place quickly and without any incapacity to work, without any annoying scars, especially when it comes to a small chronic coccyx cyst, sinus pilonidalis.

Coccyx fistula: Fistula is closed by the laser "without surgery".

Coccyx fistula: Fistula is closed by the laser beam almost "without surgery".

Procedure of an LSPC Laser Pilonidal Sinus Plastic Surgery

In the case of laser pilonidal sinus plastic surgery (LSPC), little or no incisions are generally necessary. In the HeumarktClinic we use a special ring-shaped laser systemtem, which microsurgically closes the entire fistula space and inflammatory structures without surgical incisions and scars. However, in the case of multiple, complex coccyx fistulas, this simple process must be supplemented with additional micro-slits and punches in order to be able to clear out all extensions or satellites of complex coccyx fistulas. The cyst itself is initially similar to the pit picking method  minimally invasive cleaning, especially the ingrown hair gently removed. The remaining cavity of the cyst, which still contains pus, bacteria and dead tissue, is then heated with the laser beam and the pilonidal sinus, coccyx cyst is disinfected from the inside, so that bacteria and the wall of the cyst, where the recurrences come from, come from could be vaporized together, "cooked" and then sealed. The LSPC procedure is then always adapted to the findings and individually fine-tuned based on the intraoperative findings. We also apply sutures to create compression of the cyst, skin sutures, but these are removed within 1-2 days. After that, the cyst remains closed. Suppuration can happen again, but this is rare.

preliminary examination

Before each laser pilonidal sinus plastic rehabilitation, the condition must be classified: Is it an acute or chronic coccygeal fistula? In the acute inflammatory stage, the accumulated pus must first be drained. Before a laser coccyx fistula -plastic rehabilitation, the buttocks and coccyx are examined using ultrasound and several layered images are taken. The extent, depth and content of the fistula can be determined in this way and the surgical plan can be drawn up precisely. Determining pus, hair, scars, adhesions as well as the borders and extensions before an operation is extremely important for success.

anesthetic procedures

The laser coccyx fistula plastic surgery is performed in twilight sleep and under local anesthesia. If desired, the twilight sleep can also be carried out as a short general anaesthetic. A special, cooled anesthetic solution is used for the laser pilonidal fistula plastic rehabilitation, which also serves as a cooling solution and a haemostatic and pain-relieving solution. Both the procedure itself and the local anesthetic are painless under twilight sleep.

Surgical technique of the LSPC

Before laser beams can be applied using the "keyhole technique", encapsulated hair, pus and tissue debris are sucked out through mini punches and the purulent "sinus bubble" is cleaned microsurgically. Hidden passages and fistula openings are found with a probe and prepared for the laser fiber. Acute abscesses are then sucked out, a small opening of approx. 1-2 cm is left behind for the acute inflammation to subside and other smaller micro-openings and fistulas are closed with the laser fiber and the coccyx fistula is sterilized from the inside and closed using laser coccyx fistula plastic rehabilitation. We are not talking about "surgery" because small stitches and their healing do not correspond to the burden of open surgery.

The laser beam heats and closes deep purulent ducts and fistulas in the almost completely closed procedure of the laser coccyx fistula plastic rehabilitation. Our special laser is about 0,3-0,4 mm thin and does not require any large skin openings to be inserted. The special feature of this method is the use of special cooling solutions for pain and hemostasis in one. As a result, the skin is particularly protected and heals faster than with other procedures. The locally applied pain numbing solution is still effective 5-6 hours after the operation. As a result, our patients usually have no post-operative pain and no post-operative bleeding. Depending on the extent of the coccyx fistula, the operation takes about 30-40 minutes.

After Laser Coccyx Fistula Plastic Surgery

Thanks to the gentle outpatient rehabilitation of coccyx fistulas, our patients leave the practice on their own on the same day and can start their daily work the day after next. In most cases, antibiotics are not required. Local application of cooling elements, colder compresses soaked with Rivanol promote swelling and healing. This follow-up treatment at home is also recommended in the mornings and evenings if you go to work actively. You will be seen again the day after the operation and after consultation with us.

Reviews – Effectiveness of the LSPC

The fact that we can heal almost all types and levels of coccyx fistulas using the laser pit-picking method is a fact that can be proven by statistics. Some patients also report this publicly.

As it says in the Google reviews: we cure all fistulas if the patients come back regularly for follow-up care until healing. The path is sometimes rocky when it comes to coccyx fistulas.

Patient review on Google: " I had been suffering from the consequences of a traditional coccyx fistula removal for 13 years and Dr. Haffner managed to close the wound using the laser therapy he chose. “

The abscess and cyst on the coccyx

The coccyx abscess, stone bone cyst, pilonidal sinus can be acute and chronic, long-standing and combined with a pilonidal sinus. In the acute stage of coccyx abscess is after German guidelines recommended to treat the abscess minimally invasively and to uncover and debride the abscess. This usually ensures freedom from complaints in the shortest possible time. The excision - removal - from the abscess is usually no longer necessary today, I. Iesalnieks, corresponding author1 A Ommer,2 A Herold,3 and D. Dollconsider them obsolete in their guideline on coccyx abscess and coccygeal fistula because of the significant complications. We also share this opinion: we have already taken on externally treated patients who had to live with large, open wounds for several years, because the wound healing after open coccyx cyst, pilonidal sinus, coccyx fistula operation, with the technique of fistula - cysts – Excision did not occur. We then healed the non-healing coccyx cysts with two or three revisions including laser technology and a modern wound concept in a short time.

The exposure, relief and debridement of a coccyx abscess, coccyx fistula, pilonidal sinus abscess can be performed under local anesthesia as well as general anesthesia depending on the condition of the coccyx abscess. It is noted that Dr.(H) Haffner is a proven specialist in operations under local anesthesia on all areas of the body and has 43 years of experience. We ensure that you are free of pain in any case.

There are other options for treating pilonidal sinus, coccyx cyst, as follows:

Pit Pick Op

It is a minimally invasive procedure with no laser option as a treatment alternative for chronic pilonidal sinus, coccyx cyst. However, the recurrence rate is higher compared to laser cyst surgery (LSPC). This may be due to the fact that the internal "sterilization" of the cavity with a laser beam and laser closure is omitted with this procedure. Nevertheless, one can try this option, especially for patients with restrictedtem Budget. Because the costs of the laser application are usually not or only partially covered by health insurance companies. We will then also advise you on the expected costs for both procedures.

The Limberg flap and the Karydakis procedure

are two major, radical surgical methods that are similar and consist first of extensive excision (removal) of the coccyx cyst, pilonidal sinus in healthy people. After that, however, a fairly large hole remains in the middle of the coccyx. This defect is then closed in such a way that the plastic surgeon cuts a large skin-connective tissue flap with a stalk out of the buttocks from a semicircle-like, arc-shaped incision, which is then placed on the defect area by rotating it. While the results are good, it's an op with a permanent scar that crosses the buttocks for about 10 inches.

One can ask whether one would wish for such a disfigurement today when alternative procedures are available with almost no scars. We do not need this method in any case, because we have been able to treat all cases with the LSPC method successfully.
